Purpose
Use this form to request that your research materials be reviewed and accepted for further curation by the University Library. After completing the form, you will be contacted by a library representative regarding your submission within 5 business days.

Please note that completion of this form does not guarantee that University Library staff will commit to reviewing or acquiring your research materials.

Requirements
You must be willing and have the rights to allow the University Library to take custody of, provide access to, and preserve this research material.

If you do not wish to or do not have the rights to allow the University Library to take custody of, provide access to, and preserve this research material, and have legacy computer media from which you'd like to recover content, please contact ResearchRescue@library.illinois.edu for a list of computer media data recovery vendors.
