By completing this form, you are requesting to repeat a course and to replace your old course grade with the new course grade in your GPA calculation. When you have answered all of the necessary questions, click the submit button at the bottom of the page. A form should be completed for each course you would like to repeat. Your request will be electronically submitted for review. You will receive an email notification with a final decision on your request. You should receive a response during the registration period for the term in which you would like to repeat the course. If you have any questions, please contact the AHS Student Affairs Office at 312-996-2078. IF YOU ARE NOT A STUDENT IN THE COLLEGE OF APPLIED HEALTH SCIENCES, THEN DO NOT SUBMIT THIS FORM. YOU MUST CONTACT YOUR DEPARTMENT OR PROGRAM ADVISOR IN YOUR COLLEGE TO SUBMIT THIS REQUEST.
Forms are due by the 2nd Friday of the fall and spring semesters; 1st Monday of summer session 1 and 1st Wednesday of summer session 2. Students are encouraged to submit their requests prior to the deadline dates to ensure that sufficient time is available to make registration changes in the event that a petition is denied.
All items on this form must be completed in order for your request to be processed.
Classes in which an A, B or C grade were earned cannot be repeated for GPA recalculation.
