Essential Health and Safety Criteria

Please complete this form if you would like to propose an in-person enrichment activity for Uni High students. All enrichment activities must adhere to certain essential health and safety criteria.

  • Activities must be voluntary.
  • No activities of more than 50 participants or that exceed a space's designated safety capacity.
  • For indoor activities, all students and staff are required to be tested twice in the week before an event and present an updated, negative test result on either the Safer Illinois App or Boarding Pass.
  • For outdoor activities, all students and staff are required to be tested once between 96 to 48 hours before an event and present an updated, negative test result on either the Safer Illinois App or Boarding Pass.
  • Temperature checks are required for entrance to all activities
  • Students, or members of their household, with any of the following symptoms, as outlined by the Champaign Urbana Public Health District, should not participate in any activity: fever, cough, shortness of breath or difficulty breathing, chills, fatigue, muscle/body aches, headache, sore throat, new loss of taste or smell, congestion, runny nose, nausea, vomiting, or diarrhea.
  • Masks are to be worn at all times.
  • Attendees must maintain at least six feet of social distancing.
  • Activities need to have adequate ventilation (outdoors, HEPA filters, fans, windows). Priority will be given to activities held outdoors whenever possible.
  • Sanitation guidelines are to be followed. Items should not be shared between attendees. All shared surfaces or items need to be disinfected between use.
  • Activities that involve eating and drinking are discouraged.
  • Parental permission is required.

These criteria must be met in order for any activity to receive approval.

In addition to these criteria, applicants must consider some pedagogical and practical concerns. It is essential that the experience accomplishes something that can’t be done over Zoom or some other remote learning platform. Any activity planned needs to enhance and enrich some aspect of Uni High educational experience. Teachers should be thoughtful and create an activity that justifies a family's transportation of a student to and from the activity. In these times, it is important that we respect the time and resources of our member families. 

Applications will be considered on the Monday after they are received and feedback will be given on the subsequent Wednesday. Please address any questions to the Health and Safety Task Force.
