Department of Mathematics COVID-19 Event Planning Form

Important note: This planning form is NOT a space booking form. Organizers must separately reserve the event space by contacting the person who controls it.

This form must be submitted to the chair of the Department of Mathematics for approval at least two weeks prior to the date of the event. If approval is not granted, organizers will have an opportunity to change their event plans in order to meet the policy requirements, and then resubmit.
