In the box below, describe the plant, plant problem, weed, bloom or other observation. Please provide the following information, where applicable.

  • For all observations involving plants - name of plant (if known), location (Cell 1, Cell 2 or Cell 3)
  • For plant problems - name of plant (if known), location (Cell 1, Cell 2 or Cell 3), possible causes of problem (pest, disease, drought, etc), possible solutions (if known)
  • For weed documentation - name of weed (if known), location (Cell 1, Cell 2 or Cell 3)
  • For blooming – name of plant (if known), location (Cell 1, Cell 2 or Cell 3)
  • For hardscape/infrastructure – name of element, general location (damage, pipe needs cleaned out, etc)
