COVID-19 Research Oversight Committee Intake Application

As the University of Illinois at Urbana-Champaign continues to respond to the COVID-19 pandemic, we are committed to providing agile and efficient oversight of all COVID-19 data and samples that may be used for research purposes.

 

The COVID-19 Research Oversight Committee is charged to assess, provide guidance, and make recommendations to the Vice Chancellor for Research and Innovation on requests for access to the University’s COVID-19 data and related biological samples. Please complete the form on the next page to initiate your request with the Illinois National COVID-19 Registry.

 

Types of Data/Samples Available*

  • Data: Positive/Negative Diagnosis
  • Samples: Saliva (heat-inactivated, frozen)

* All data/samples are currently de-identified.

 

Application Requirements

  • Investigator Information: Includes name, contact information, and institutional affiliation of the principal investigator. In most cases, the principal investigator must have a Ph.D., J.D., or M.D. degree and be affiliated with an academic or research institution.
  • Research Staff Information: Include name, contact information, institutional affiliation, and expertise of all research staff members who will access the data. All research staff members must have required trainings prior to being provided access to data/samples.
  • Research Description: Applicants will submit a research description which includes a description of the research project that the data/samples will be used for and clear information about why these specific data/samples are being requested.
  • Confidential Data Security Plan: Applicants will detail a confidential data security plan. The fundamental goals of this plan are to ensure that the data/samples are stored securely and are accessible only to the people listed in the application.
  • IRB/IBC Approval: Applicants must provide documentation of IRB approval and/or IBC registration for the proposed research project, if necessary.
  • Other Requirements: Some uses of data/samples have additional requirements, such as obtaining special certifications or filling out additional forms.

 

Application Review and Approval Process
After an application is submitted, it is reviewed by C-ROC staff. The amount of time it takes to review an application depends on whether and how many changes are required before a determination. C-ROC staff will work with the requester until all required information is provided and the request can be reviewed at a meeting. Generally, it takes 2-4 weeks for a determination to be made on an application after it is submitted.

 

Application Maintenance
After an application is approved, it is the Investigator’s responsibility to ensure the data/samples are being used in a manner consistent with the approved application.
